**READ FIRST**  In an effort to keep your information secure we have opted NOT to collect the following information via this form: Your SS# and your Payment information.  We will contact you via phone to collect this information.  In addition, a completed W-9 form must be received by the home office within 15 days of application approval.  Consultant manuals, if ordered, will not be mailed out until the W-9 form has been received by the home office.

Kits will begin shipping in February 2007.

No Kit

I would like the FREE sign up option with no kit at this time.  A consultant manual is the only provision with this option.  I understand it will be available in the Consultant Area of the website to be printed by me.

 

 

Terms & Conditions

I understand that the terms and conditions of this application are included in the terms, conditions and guidelines outlined in the consultant manual.  I further understand that these terms and conditions may be amended in writing from time to time.

 I understand that I have a 10 day period, from the date listed above,  to cancel this agreement without penalty.  However, in the event that the kit has shipped out before cancellation, or if cancellation occurs after the 10 day period, there will be a 25% restocking and replenishing fee deducted from my refund.  This fee is to cover the replacement of consumable and unusable products that are contained within the kit.   I understand that there is a no return, no refund, no exchange policy on all food items.

 I agree that the above information is complete and accurate and that I am a legal U.S. Citizen  or resident over the age of 18.

 I understand and agree that I am an Independent Contractor and that I am not an employee of Sweet Wrap Workz or its parent company, The Chocolate Works & More.  As an Independent Contractor I understand and agree that I am not entitled to worker’s compensation, insurance, tax deductions, etc that go along with employment.  I am solely responsible for reporting and remitting any local, state and federal taxes due including, but not limited to, self-employment taxes, sales and use taxes, etc that accrue and become due in relation to my commissions, bonuses, prizes, and any other compensation received from Sweet Wrap Workz.    I also understand and agree that my earnings are solely based upon my efforts to sell and market the products to customers.  I agree to comply with all local, State and Federal rules and regulations pertaining to Independent Contractors.  I agree that I will state clearly in all advertisement that I am an Independent Consultant/Contractor.  And all advertisement will also bear my Independent Consultant Identification Number.

 I agree that I am to sell through personal sales , online sales, catalog sales, mail orders, and home office leads.  I understand that I may not sell Sweet Wrap Workz products side by side with any other product lines.  I will always represent the company in a true and positive manner reflecting the high standards of ethics and integrity that Sweet Wrap Workz is founded on.  I also understand that I may not re-label any products but may add discrete labels with my name, contact and consultant information so long as it does not obscure any Sweet Wrap Workz labeling or contact information.  I also agree to the Non-Compete terms of this application in that I will not represent any other competing company at the same time as this contract is in force.

 I understand that all information and materials (including, but not limited to: Business Opportunity Guide, Newsletters, website, mailing list, etc) provided to me by Sweet Wrap Workz is to be handled as Confidential and Proprietary Information belonging solely to Sweet Wrap Workz.  I agree that I am not to disclose or reproduce any information obtained through Sweet Wrap Workz except as needed for the operation of my business as an Independent Contractor with the company.

 I agree that I will not contact any suppliers of Sweet Wrap Workz.  And upon termination of this agreement I will not only abstain from contacting known suppliers of Sweet Wrap Workz, but I also agree that I will not use any knowledge or information obtained during my time as a Sweet Wrap Workz Independent Consultant, or information obtained by independent research,  to start any such business that would be considered to be in direct competition with Sweet Wrap Workz for a time period of no less than 2 years from the date of official termination.  I also agree that upon termination of this agreement that I will pay any outstanding balances due to Sweet Wrap Workz.  And that Sweet Wrap Workz has the authority to deduct any monies owed to them by me from my final commission check, if any, before it is mailed to me.
